Mal\u00e1ri\u00ed ka\u017edoro\u010dn\u011b onemocn\u00ed miliony lid\u00ed, zejm\u00e9na v tropick\u00fdch a subtropick\u00fdch oblastech. Jedn\u00edm z nejzaj\u00edmav\u011bj\u0161\u00edch p\u0159\u00edklad\u016f je <strong>Duffy antigen<\/strong>, specifick\u00e1 molekula na povrchu \u010derven\u00fdch krvinek, kter\u00e1 m\u016f\u017ee hr\u00e1t kl\u00ed\u010dovou roli p\u0159i infekci mal\u00e1ri\u00ed zp\u016fsoben\u00e9 jedn\u00edm konkr\u00e9tn\u00edm druhem parazita. Jak m\u016f\u017ee pr\u00e1v\u011b tento genetick\u00fd detail ovlivnit riziko infekce?<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Co je to mal\u00e1rie a jak vznik\u00e1?<\/strong><\/h2>\n\n\n\n<p>Mal\u00e1rie je infek\u010dn\u00ed onemocn\u011bn\u00ed zp\u016fsoben\u00e9 mikroskopick\u00fdmi parazity rodu Plasmodium. Na \u010dlov\u011bka se p\u0159en\u00e1\u0161\u00ed po bodnut\u00ed infikovanou sami\u010dkou kom\u00e1ra rodu Anopheles. Nejprve se dostane do jater, kde se v bu\u0148k\u00e1ch n\u011bkolik dn\u00ed a\u017e t\u00fddn\u016f mno\u017e\u00ed, ani\u017e by na sebe upozor\u0148oval. Teprve po t\u00e9to tich\u00e9 f\u00e1zi se paraziti uvoln\u00ed do krevn\u00edho ob\u011bhu a za\u010dnou napadat \u010derven\u00e9 krvinky (erytrocyty). Pr\u00e1v\u011b tato f\u00e1ze je zodpov\u011bdn\u00e1 za typick\u00e9 projevy mal\u00e1rie, jako jsou opakuj\u00edc\u00ed se hore\u010dky, zimnice, an\u00e9mie nebo v\u00fdrazn\u00e1 \u00fanava.<\/p>\n\n\n\n<p>V\u011bdci dnes znaj\u00ed p\u011bt druh\u016f parazit\u016f rodu Plasmodium, p\u0159i\u010dem\u017e nejz\u00e1va\u017en\u011bj\u0161\u00ed formy onemocn\u011bn\u00ed zp\u016fsobuje Plasmodium falciparum, kter\u00fd je roz\u0161\u00ed\u0159en zejm\u00e9na v subsaharsk\u00e9 Africe. Druh <strong>Plasmodium vivax<\/strong>, na kter\u00fd se zam\u011b\u0159uje i genetick\u00fd mechanismus sledovan\u00fd v na\u0161\u00ed anal\u00fdze, je naopak nejroz\u0161\u00ed\u0159en\u011bj\u0161\u00edm p\u016fvodcem mal\u00e1rie mimo Afriku.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong><strong>Kdy genetika ovliv\u0148uje infekci mal\u00e1ri\u00ed?<\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p>V\u011bdci dnes st\u00e1le \u010dast\u011bji zji\u0161\u0165uj\u00ed, \u017ee u tohoto onemocn\u011bn\u00ed nehraje roli pouze prost\u0159ed\u00ed, ale tak\u00e9 genetika \u010dlov\u011bka. N\u011bkter\u00e9 genetick\u00e9 varianty toti\u017e mohou ovlivnit, zda se parazit dok\u00e1\u017ee dostat do na\u0161ich \u010derven\u00fdch krvinek.<\/p>\n\n\n\n<p>Jedn\u00edm z takov\u00fdch p\u0159\u00edklad\u016f je tzv. <strong>Duffy antigen<\/strong>, molekula na povrchu \u010derven\u00fdch krvinek, kterou parazit Plasmodium vivax vyu\u017e\u00edv\u00e1 k vstupu do bu\u0148ky. V na\u0161\u00ed DNA anal\u00fdze sledujeme genetickou variantu v <strong>genu ACKR1<\/strong>, kter\u00e1 m\u016f\u017ee ovlivnit tvorbu tohoto antigenu.<\/p>\n\n\n\n<p>Pokud se Duffyho antigen na \u010derven\u00fdch krvink\u00e1ch nevytv\u00e1\u0159\u00ed, m\u016f\u017ee m\u00edt parazit Plasmodium vivax v\u00fdrazn\u011b zt\u00ed\u017een\u00fd vstup do bun\u011bk. To m\u016f\u017ee poskytovat p\u0159irozenou ochranu p\u0159ed t\u00edmto typem mal\u00e1rie.<\/p>\n\n\n\n<p><strong>Je v\u0161ak d\u016fle\u017eit\u00e9 zd\u016fraznit, \u017ee se jedn\u00e1 o specifick\u00fd mechanismus t\u00fdkaj\u00edc\u00ed se pouze jednoho druhu parazita, nikoli o ochranu p\u0159ed v\u0161emi formami mal\u00e1rie.<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong><strong><strong><strong><strong>Pro\u010d je dobr\u00e9 zn\u00e1t tuto predispozici?<\/strong><\/strong><\/strong><\/strong><\/strong><\/h2>\n\n\n\n<p>Mal\u00e1rie je v\u00fdsledkem slo\u017eit\u00e9ho biologick\u00e9ho cyklu mezi kom\u00e1rem, parazitem a lidsk\u00fdm organismem. V jedn\u00e9 z jej\u00edch nejd\u016fle\u017eit\u011bj\u0161\u00edch f\u00e1z\u00ed, p\u0159i napaden\u00ed \u010derven\u00fdch krvinek, m\u016f\u017ee hr\u00e1t roli i genetick\u00e1 v\u00fdbava \u010dlov\u011bka.
